PROCEDURE FOR REHABILITATION ENGINEERING REFERRALS
 
            In order for our facility to provide the best possible service to your client, it is important for us to have complete    information.  Below are listed our referral information requirements.  All of these forms will be provided by the Center.

  Referral Procedure for Rehabilitation Engineering Clients
 

 1.     Call Engineer regarding adaptive equipment/modifications

 2.     Prepare Referral Packet
        a.    Referral Form for Rehabilitation Engineering Field Services
                            - include a statement of problem to be addressed.
        b.    Include Relevant Case File Information

 3.     Send the referral to the Regional Office Coordinator for Field Services

 4.     Coordinator will approve and prioritize referrals, and send packet on to engineer.

 5.     Counselor should plan to make initial client visit with the engineer.
